
Breastfeeding rates in the UK are consistently low, campaigners and NGOs are frustrated at policymaking inertia, and current research shows inequity of access to breastfeeding support; both based on levels of disadvantage and ethnic background. Impact on Urban Health commissioned Bremner & Co to undertake a comprehensive policy and practice review to understand these challenges and guide their role in addressing systemic barriers. The review aimed to amplify the voices of those shaping the system, from health practitioners to policymakers, and uncover opportunities for meaningful change.
Our Approach
We conducted an in-depth analysis involving extensive stakeholder engagement and data synthesis. We held interviews with health visitors, local authorities, campaigners, midwives, and NGOs to capture their diverse perspectives on breastfeeding barriers, including cultural considerations, funding gaps, and workplace challenges. This qualitative research was paired with a review of existing academic and grey literature to ensure a well-rounded understanding of the landscape. By navigating a complex and sensitive topic, we provided a nuanced and actionable perspective on the systemic inequities affecting breastfeeding outcomes.
Key Outcomes
Our breastfeeding landscape review revealed systemic inequities and sparked critical conversations about potential interventions, emphasising the urgent need for improved policy, funding, and cultural support for breastfeeding mothers, particularly in disadvantaged communities. Our findings contributed to wider advocacy efforts, including the Food Foundation’s ‘Barriers to Breastfeeding’ report and in their podcast series.

Whilst the academic and grey literature told us a lot about why women are being failed by the breastfeeding support system – it was hearing from those on the ground that brought such a richness to this piece of work.
